Care managers are healthcare professionals who act as central coordinators, assessing a patient's needs, developing personalized care plans, and connecting patients with necessary services like doctors, therapists, and community support systems. Care managers play a crucial role in helping individuals and families navigate eldercare. Attend this panel discussion to learn more about how you can assist your clients with care management and about how to find a care manager, as well as to gain a deeper understanding of issues related to eldercare.

Jennifer Szakaly, MA, CMC, NMG, opened the Charlotte-based geriatric care management and counseling firm, Caregiving Corner, in 2005 to assist caregivers of older adults in navigating the long-term care system and to arm them with the knowledge they need to make informed decisions when dealing with a crisis caregiving situation. Her specialties include assessments, referral and placement services, family mediation, care plan development, VA benefits application assistance, mediation application assistance, service coordination, monitoring of long-distance caregiving situations, and crisis intervention.
